STATEMENT OF NON-SHARING FOR MOBILE NUMBERS End-user agrees that any mobile phone number(s) provided by them to PFRANKMD shall be unique to that end-user. User agrees mobile number(s) provided to PFRANKMD are not shared with, or common to, other end-users. If end-user changes mobile phone number(s), it is end-user’s responsibility to contact PFRANKMD to update their mobile phone number in a timely manner.
