A wrinkle is a fold or crease in the skin. Skin wrinkles typically result from age, genetic predisposition and most importantly sun damage. Smoking, dry skin, excessive motion, poor hydration, and several other factors may contribute. Although there are numerous topical options on the market, few have any significant efficacy compared to procedural means. Contrary to popular belief, a facelift does not treat wrinkles, only loose skin. There are several dermatologic treatments using injectables and laser treatments to dramatically improve their appearance.

Treatments for Wrinkles and Fine Lines at PFRANKMD
The Miria represents the future of non-ablative laser skin tightening for scars, fine lines, and wrinkles. It introduces a new approach to non-ablative fractional resurfacing.
Fillers such as Silicone, Radiesse, Juvéderm, Restylane, Sculptra, and Perlane are used to lift an indentation that wrinkles cause. Fillers can also be used to lift the skin for a facelift type effect. Done non-invasively this is called a filler facelift or FLT procedure.
Botox and Dysport are FDA-approved muscle relaxers that address wrinkles associated with motion.
For a few decades now, lasers have been used to resurface wrinkles on the face and body. Currently, the state of the art technology is Fractional Skin Resurfacing.
Ellacor is the first device of its kind to non-surgically remove wrinkles while lifting and tightening skin. By utilizing rapidly automated fine needle extractions of skin throughout the area of treatment, the device removes eight percent of skin per session under local anesthesia.
The EMFACE Under Eye applicators are a revolutionary treatment that leverages high-intensity muscle stimulation and radiofrequency energy to tighten, lift, and smooth the under-eye region.

Achieving the best possible outcome from facial plastic surgery extends far beyond the operating room. Post-surgical care plays a critical role in supporting healing, enhancing results, and maintaining facial harmony over time. Immediate postoperative management focuses on controlling swelling and bruising through modalities such as lymphatic drainage, cold compresses, and, when appropriate, vascular laser treatments like VBeam. These therapies can minimize inflammation, reduce discoloration, and accelerate visible recovery.
Ensuring that incisions heal cleanly and without complications is also essential. This involves proper wound care, scar prevention strategies, and sometimes the early use of treatments like silicone gels or light-based therapy to minimize scar visibility. Neuromodulators (such as Botox or Dysport) may also be used in the early healing stages to reduce tension on healing incisions and prevent the formation of dynamic wrinkles.
As healing progresses, the focus shifts toward refinement and long-term maintenance. Cosmetic injectables—including hyaluronic acid fillers, biostimulatory agents like Sculptra, and neuromodulators—can help restore volume, smooth fine lines, and enhance surgical outcomes without the need for further invasive procedures. These treatments are especially effective in maintaining youthful contours and prolonging the benefits of surgery.
Ultimately, collaboration between a skilled facial plastic surgeon and an experienced cosmetic dermatologist ensures a 360-degree approach to rejuvenation. By combining surgical precision with advanced aesthetic maintenance, patients can enjoy natural-looking, long-lasting results that evolve beautifully over time.

𝐅𝐢𝐥𝐥𝐞𝐫 𝐅𝐞𝐚𝐫: Reversing Filler and Understanding the Pros and Cons of Filler Reversal
When considering injectable fillers, it’s important to understand the reversibility of each type. Hyaluronic acid fillers, for example, can be reversed using hyaluronidase. However, it’s crucial to be aware of the associated risks and considerations when opting for filler reversal.
Choosing the right filler type should be based on a careful assessment of safety, aesthetic goals, and longevity, considering various strategies to achieve the desired outcome.

𝐈𝐬 “𝐆𝐋𝐏’𝐝” 𝐭𝐡𝐞 𝐍𝐞𝐰 𝐒𝐧𝐚𝐭𝐜𝐡𝐞𝐝?
Two years after I coined the term “Ozempic Face,” I’m seeing the next evolution in aesthetics.
This time, the trend is called “GLP’d” (pronounced Glypped).
Rapid weight loss on GLP-1s led to facial hollowing and premature aging, what I called Ozempic Face. What I’m seeing now is very different.
Today, I’m observing normal-weight individuals using low-dose GLP-1s not primarily for weight loss, but for reducing systemic inflammation, excess water weight, and bloating.
As inflammation and fluid retention go down, facial swelling decreases. The result is subtle but noticeable: sharper contours and more visible natural bone structure.
The drug helps define what’s already there. When inflammation and fluid retention go down, cheekbones, jawlines, and facial angles naturally emerge without over-reliance on fillers.
Unlike Ozempic Face, which stemmed from excessive fat and volume loss, being GLP’d reflects controlled refinement on an already lean frame, with a focus on natural geometry rather than artificial enhancement.
I’m also seeing the rise of “GLP’d and Ripped,” where patients pair GLP-1 therapy with protein optimization, strength training, creatine, and peptide protocols to preserve muscle mass while improving overall definition.
This is an aesthetic and metabolic shift.
It’s about clarity, structure, and balance.
One of the most common and problematic areas to treat on the face is the zygomatic fat pad. This is the region between the lower eyelid and the cheekbone.
This area is generally unreachable by surgical intervention and we want to avoid injectables in that region because they can potentially make the area protrude out more.
In the past, we have attempted several treatments of microneedling and heat based devices to improve the area, with limited success.
Through the use of fractional micro-coring, we are able extract portions of the skin minimally invasively, with more effectiveness and less scarring.
We are continuing to see incredible results with the Ellacor technology, any part of the face and body, including severe wrinkles in the mid to lower face, jowls, the area under the neck, the abdomen, elbows, and knees.

At the Complication Corner, I discuss a case featuring a patient who suffered second-degree burns with blisters from an IPL device at a med spa chain. Unlike controlled first-degree burns from properly administered laser treatments, this procedure was performed without healthcare professional supervision or medical oversight.
I explain how proper wound care and follow-up helped achieve healing, though some pigmentation irregularity remains, which we plan to treat with non-ablative Pico lasers and Fraxel Thulium.
This case highlights the serious risks of getting laser and light-based procedures done at med spas without appropriate medical supervision, as these are controlled burns that require professional expertise.
Fraxel is one of the most advanced laser technologies for treating stretch marks—both new, red marks and older, faded ones.
✨ How it works: Fraxel creates microscopic columns of controlled injury in the skin, stimulating collagen production and encouraging the skin to remodel itself. This improves both the color and the texture of stretch marks, making them less visible over time.
📅 On average, it takes 3–6 treatments for noticeable improvement, though the exact number depends on the depth, age, and type of stretch mark. Consistency is key—stretch marks that are older or deeper often require more sessions.
📊 Results shown here are after 5 Fraxel treatments. Each patient is unique, and the best outcomes often come from a customized plan that may combine Fraxel with other cutting-edge technologies available at PFRANKMD.
At PFRANKMD, our team takes a science-backed approach to create personalized solutions that deliver long-term beauty and confidence.

Our patient shares her experience just 24 hours after her first Ellacor micro-coring treatment, highlighting how comfortable the procedure felt and why she chose Ellacor to tighten and sculpt her face with minimal downtime.
Erin, 45, describes the treatment as virtually painless, noting only a mild sunburn-like sensation 24 hours after the treatment. Her goal was natural-looking tightening and improved facial contour without surgery. At just one day post-procedure, she already felt confident about her decision and impressed by how manageable the recovery has been.
Planning for an event? While Ellacor has significantly less downtime than traditional surgical options, optimal results develop gradually as the skin rebuilds collagen and elastin. For best results, we recommend starting Ellacor at least 8–12 weeks before a major event, allowing time for healing, collagen stimulation, and refinement of results.
Some patients may benefit from a series of treatments depending on their skin goals and timeline.
Ellacor is ideal for patients seeking meaningful skin tightening with natural results. Call (212) 327-2919 or click the link in bio to schedule a consultation.

Fat transfer was once a foundational part of my practice, before the rise of fillers and biostimulatory agents. Now, with advances in microfat and nanofat processing, fat-based treatments are experiencing a powerful resurgence and for good reason.
Fat is one of the most accessible and regenerative resources in the human body. Rich in stem cells and growth factors, it offers remarkable potential not only in aesthetics, but across regenerative medicine including orthopedic and cardiac applications. These innovations allow us to harness fat’s natural healing properties in a more refined, precise way, supporting skin quality, tissue regeneration, and long-term vitality.
As we move into 2026, I am increasingly focused on helping patients age better naturally, optimizing their appearance and health while minimizing reliance on foreign substances. This evolution reflects a broader shift toward regenerative, whole-body approaches that align beauty with longevity.
I am deeply grateful for my career, my patients, and the opportunity to continue advancing medicine in a way that supports natural beauty, resilience, and pro-aging from the inside out.
