Hyperpigmentation

Hyperpigmentation is a very common condition that many people suffer from, at all ages. This discoloration of the face gives patients an uneven skin tone, leaving many patients frustrated and self-conscious about their appearance. It is most commonly caused by overexposure to the sun, but can also be caused by almost any disruption to the skin, such as acne or a rash. Hyperpigmentation can be difficult to treat with at-home or over the counter remedies, especially for patients with stubborn and extensive situations. However, the appearance of hyperpigmentation can often be greatly reduced by treatments available from your expert dermatologists at PFRANKMD by Dr. Paul Jarrod Frank.

What Is Hyperpigmentation?

Hyperpigmentation is defined as discoloration of certain areas of the skin, leading to dark spots and uneven skin tone. This is due to an overproduction of melanin, the natural pigment found in skin which determines everyone’s eye, hair, and skin color. There are many contributing factors to a patients’ likelihood of hyperpigmentation. The most common cause is sun exposure, but other common contributors are hormonal changes, such as pregnancy, acne, allergies, and other trauma to the skin. People with darker skin tones are more prone to hyperpigmentation.

Because there are so many potential causes of hyperpigmentation, many people suffer from this condition, and thus there are many attempts to treat it. There are over the counter and at home treatments such as topical creams and serums which may include hydroquinone, specific acids, or antioxidants. Avoiding the sun and using skincare products is critical during hyperpigmentation treatments.

Each case is unique, as the cause and severity of the condition, the patient’s skin type, and desired results can affect which treatment is the best option for each patient.

Depending on the severity and location of hyperpigmentation, your dermatologist will create a personalized treatment plan that may include Fraxel laser treatments.

For best results, it is important to take care of your skin post treatment. Many patients choose to undergo yearly maintenance treatments, or at a frequency discussed with our doctor. It is also vital to limit sun exposure and wear sun protection at all times, to prevent further hyperpigmentation and protect the skin.

We discuss the important preparatory steps and precautions to take before undergoing skin resurfacing or restructuring procedures. This includes avoiding prolonged sun exposure, limiting use of potentially irritating skincare products such as retinoids, alpha hydroxy acids (AHAs), and controlling any active skin inflammation.

We also cover the use of lightening agents like tranexamic acid, which can be administered orally or topically, may be recommended to help diminish hyperpigmentation and improve overall skin tone prior to the procedure.

The recovery process and continued sun protection post-procedure is vital to prevent complications and maintain results.

At the Complication Corner, I discuss a case featuring a patient who suffered second-degree burns with blisters from an IPL device at a med spa chain. Unlike controlled first-degree burns from properly administered laser treatments, this procedure was performed without healthcare professional supervision or medical oversight.

I explain how proper wound care and follow-up helped achieve healing, though some pigmentation irregularity remains, which we plan to treat with non-ablative Pico lasers and Fraxel Thulium.

This case highlights the serious risks of getting laser and light-based procedures done at med spas without appropriate medical supervision, as these are controlled burns that require professional expertise.
Fraxel is one of the most advanced laser technologies for treating stretch marks—both new, red marks and older, faded ones.

✨ How it works: Fraxel creates microscopic columns of controlled injury in the skin, stimulating collagen production and encouraging the skin to remodel itself. This improves both the color and the texture of stretch marks, making them less visible over time.

📅 On average, it takes 3–6 treatments for noticeable improvement, though the exact number depends on the depth, age, and type of stretch mark. Consistency is key—stretch marks that are older or deeper often require more sessions.

📊 Results shown here are after 5 Fraxel treatments. Each patient is unique, and the best outcomes often come from a customized plan that may combine Fraxel with other cutting-edge technologies available at PFRANKMD.

At PFRANKMD, our team takes a science-backed approach to create personalized solutions that deliver long-term beauty and confidence.

Linea nigra is a form of hormone-related hyperpigmentation that commonly appears during pregnancy and may remain long after delivery. While it can fade naturally over time, lingering pigment is common once hormones begin to stabilize postpartum.

Pico laser treatments help improve discoloration by breaking up stubborn melanin without excess heat, making it a safe and effective option for postpartum skin. Treatments are gentle, require minimal downtime, and support overall skin clarity and texture with gradual, natural-looking improvement.

Results build with a series of sessions and are best supported with consistent sun protection and medical-grade skincare to help maintain an even tone.

Our patient shares her experience just 24 hours after her first Ellacor micro-coring treatment, highlighting how comfortable the procedure felt and why she chose Ellacor to tighten and sculpt her face with minimal downtime.

Erin, 45, describes the treatment as virtually painless, noting only a mild sunburn-like sensation 24 hours after the treatment. Her goal was natural-looking tightening and improved facial contour without surgery. At just one day post-procedure, she already felt confident about her decision and impressed by how manageable the recovery has been.

Planning for an event? While Ellacor has significantly less downtime than traditional surgical options, optimal results develop gradually as the skin rebuilds collagen and elastin. For best results, we recommend starting Ellacor at least 8–12 weeks before a major event, allowing time for healing, collagen stimulation, and refinement of results.

Some patients may benefit from a series of treatments depending on their skin goals and timeline.
